A multi-agency
pilot project which is being led by Coventry Social Services will give older people who
are discharged from hospital up to six weeks intensive care packages aimed at helping them
to stay in their own homes.
To begin with the project, which also involves Coventry Health Authority and the
Community Trust, will be limited to patients who fall within the pilot scheme area, but if
it is successful it will be extended throughout the city.
The development comes hot on the heels of the Government white paper on Social Services
and fits in with the aim of extending independence in later life.

Ann Lucas, Chair of Coventry City Council's Health & Care Policy Team, said:
"All too often older people are discharged from hospital directly into residential
care which is where they remain. We believe that with the right type of intensive support
many of them could be helped to stay in their own homes.
"Re-ablement will help people to maintain their independence in their own homes by
providing a range of support, not only to help physical recovery put also to promote the
self confidence and self esteem.
"This project recognises the vital part played by family, friends and neighbours
in the recovery and independence of many older people. This support will be integrated
into the package along with the professional care, to allow as many people as possible to
lead independent lives in their own homes."
